Please use the following form to submit prayer requests. This form will go to the chairperson of the Prayer Chain. If you have marked it to be added to the praying hands, it will be , otherwise it will not. None of the information is required, the fields are just there as suggestions of what to include. This form will be sent using the default mail program on the computer being used to submit it. It does require that you have a mail account active on that machine.
